Imagine You & Me (IYAM) director Mike Tuviera briefly explained the reason why Alden Richards and Maine Mendoza took five takes to get the hospital scene in IYAM right.
Direk Mike revealed that it was because of the heart rate monitor which they attached to Maine.

I blocked the scene and we rehearsed it a couple of times for camera. No acting yet, no emotions. Just for framing.

Direk Mike added that Maine's heart beats faster every time Alden would come near her.
But during the take, whenever Alden would stand up & move slowly towards Maine's face, the heart rate monitor would start beeping faster.

The staff had a hard time keeping their emotions in check as Alden was crying in the scene. In the end, Direk Mike said he captured a "beautiful end to a very sad scene."
